Live Well, Be Well Prevention Plan




Live Well, Be Well Prevention Plan for CD PPO HDHP Participants



The Live Well, Be Well Prevention Plan provides eligible participants with a personalized, confidential wellness program to get healthy and feel better…with a variety of tools and personal attention throughout the year.  It is designed based on the best medical research.

The program focuses on three key elements:

  • Baseline Assessment: this assessment determines your current health profile and is derived from the results of your blood test and responses to the Health Risk Appraisal.

  • Intervention: this component integrates the results of your current health profile (baseline) with intervention strategies aimed at improving health, and preventing the onset of illness and disease.

  • Live Well, Be Well Prevention Plan:  The personal prevention plan report describes current health status and the actions one can take to maintain and improve health. Members also have access to their very own secure website containing healthy actions, planned community events and challenges, secure online data storage for lab tests and medical records, appointment reminders, and a medical library that contains reliable information on hundreds of topics on health conditions, diseases and wellness.

Participants who have completed the required steps and took healthy actions during Plan Year 2011 can earn up to $360 off health insurance premiums for Plan Year 2012, beginning July 1, 2011. Inside each member's Prevention Plan website is their own confidential Prevention Score that helps track their individual prevention efforts and progress. The Intervention component of the Score will fall into one of four levels (each in 125-point increments). On May 31, 2011, the level of your Prevention Score will determine the premium reduction effective July 1, 2011.

How Can I Improve My Prevention Score?

  1. Visit your online Prevention Plan consistently to learn about what prevention actions you need to take.
  2. Take those recommended prevention steps throughout the year.
  3. Update information about your actions in your Prevention Plan throughout the year.

For example, if you have a strong family history of colon cancer or are at least 50 years old, and you are due for your colonoscopy, the Prevention Plan will make this recommendation.  Completing and recording this recommended activity will increase your score for that membership cycle.  Similarly, if you have are at high risk for heart disease, your Prevention Plan will recommend you that complete Action Programs related to your risk.  This also will increase your score.

Together with following other recommendations – such as exercising, eating healthy, learning about your other risk factors and acting upon the strategies that can keep you healthy – you will continue to improve your Prevention Score each year, and in the process your overall health as well. 

Is my Prevention Score based on my health status?

The Prevention Score evaluates your effort and whether you are taking care of yourself, rather than where you are right now in terms of your health. This means you can have a high score even if you have a medical condition such as heart disease, diabetes or high blood pressure.

The Prevention Score also does not penalize you for something that is beyond your control, such as your genetic predisposition to certain health conditions. If you have been diagnosed with a condition that could impact a portion of your score and it cannot be modified by lifestyle changes, you will be given the maximum score in the category this affects. Does a high Prevention Score mean I am in good health?

A high Prevention Score does not guarantee better health. Rather, it shows that you are involved in certain prevention activities that have been shown to reduce chances of a bad health outcome, when compared to someone with a low Prevention Score.

Who gets to see the results of my Prevention Score?

No one will see your personal information except the health professionals at The Prevention Plan. The results of your Prevention Score are completely confidential and will never be shared with your employer, your insurance provider or anyone else.

Can my score be used against me?

No, because your exact score will never be shared with anyone, including PEBP. In fact, by taking small steps to consistently improve your health you may actually see some savings such as less medical and out-of-pocket costs.

When will I find out my final Prevention Score?

Your final Prevention Score will be provided after completing 12 months of The Prevention Plan and completing a year-end assessment. Of course, you can see your current score at anytime by logging on to The Prevention Plan and clicking on My Prevention Score.
