Wellness Program Wellness Program Self-funded PPO Plan The Self-funded PPO Plan offers each participant an in-network Wellness benefit for healthcare services that are not provided as a result of illness, injury, or congenital defect. Under the Wellness Program, the plan year maximum is $2,500 per person; services are not subject to deductible and are payable at 100%. Family Health History Tool The Surgeon General of the United States announced the release of the Family Health History Tool available via web for people to enter their family history and read user friendly diagrams and graphics specific to their family and the data they enter. “Family history has always been an important part of good health care, but it has been underused,” said Acting Surgeon General Steven Galson, a rear admiral in the U.S. Public Health Service. “Today, with our growing knowledge of genetics, family history is becoming even more important. The new tool will help consumers and clinicians alike. It will also serve as a platform for developing new risk assessment software that will help in screening and prevention of cancer, heart disease, diabetes, and other conditions.” One organization saying it will link to the new tool is the Lance Armstrong Foundation (LAF), a cancer advocacy organization. “A strong family health history tool can be an important element for guiding medical decision-making, especially in the area of cancer screening, prevention and early detection,” said LAF founder and chairman Lance Armstrong. “This tool will further the capabilities of electronic health records and takes a significant step toward improving clinical care.” You can find the Family Health History Tool here: https://familyhistory.hhs.gov
Wellness Fair The Public Employees' Benefits Program offers Wellness Fairs to all PPO and HMO Plan participants age 18 years or older. The fairs are designed to provide participants and covered dependents with convenient health screenings and information related to healthy living For entry into a Wellness Fair, please remember to bring your PPO or HMO Plan medical ID card. Entry times at larger fairs are determined by the participant's last name. If you have questions,

At Health Plan of Nevada, we provide programs dedicated to promoting lifelong wellness. The Health Education and Wellness Division offer a variety of health education classes and disease management programs for HPN participants. For more information or to register for a class, please call HPN at 702-877-5356 or 800-720-7253 or visit HPN. 2008- 2009 Cardiac Wellness Pilot Program (CWP) (By Invitation Only) PEBP kicked off the second phase of its Cardiac Wellness Program (CWP) in April, 2009. Enrollment in the CWP was offered to 2,000 randomly selected PPO participants only. The purpose of the CWP Pilot Program is to help participants better understand and reduce their risk of developing heart disease through the adoption of healthy lifestyle behaviors and control of risk factors if present.
